Housing & Dining Cancellation Request
Follow these instructions to submit a Housing Application Cancellation Form
- Log in to the housing portal.
- Go to the tab titled “Applications and Forms.”
- Click on the “Application Name – Cancellation” form.
- Complete the information on the form and attach any applicable documents.
- If you are withdrawing, transferring, graduating, on track to earn over 60 credits or will be 21 years of age or older before the start of the semester, no additional documents will need to be attached.
- If the above do not apply, you will need to print, complete, sign and attach the CLPA or RFR form to your cancellation form submission.
- Certification of Living with Parent (CLPA) Form (Complete this form if you are living with a parent/legal guardian within a 40-mile commuting distance from campus.)
- Request for Release from Residency Requirement (RFR) Form (Complete this form for all other circumstances that you believe would allow you to be exempt from the housing requirement.)
Please allow 1-3 business days for processing. Housing & Dining staff will notify you when the form is processed.
